Streamlining care coordination and bridging communication gaps, we reduce the stress of navigating complex healthcare systems, allowing patients to focus on their health, well-being, and future outcomes.
We are dedicated to ensuring smooth transitions, and streamlined care. Medical Navigation Services helps alleviate the stress of navigating complex healthcare systems, representing both patients and administrations. We provide comprehensive support to patients, healthcare teams, and their respective representatives, ensuring optimal health and organizational outcomes. Acting as a bridge within healthcare systems, we address gaps in patient follow through and fragmented processes by:
- Representing patients and non-patients, regardless of insurance, age, or health condition
- Referral monitoring and follow through
- Medication management
- Transitional care management for hospital-to-home
- Assisting with scheduling, transportation, and temporary housing while receiving out-of-area care
- Identifying emergency funding resources for patients in crisis
- Assisting non-patients with resources, research, and preventative plans
- Assistance with long-term planning for those with chronic conditions or proactive aging
- Educating around POLST and Advance Directives
- Notarial services for medical documents
Fee-based Services: Our partners donate up to 20% of fees incurred back to Medical Navigation Services. Fee-based services include:
- Non-medical notary public: Including and not limited to contracts and agreements, property deeds, loan and mortgage documents, affidavits, powers of attorney, wills and trusts, business documents (such as articles of incorporation), adoption papers, and court documents. **Medical and Advanced Care Planning notarizations are zero cost
- Medicolegal Nurse Consultation for legal firms and similar entities (not available for patient or otherwise private consult)
- Organizational process monitoring and professional coaching: Business planning, leadership development, and operational audits to improve organizational efficiency and change management
- Corporate succession planning
- Financial planning for long-term financial stability
Medical Navigation Services relies upon a robust network of volunteer expert advisors, including board-certified physicians, credentialed specialists, nurses, legal representatives, financial and business consultants, social workers, and spiritual advisors, A California Notary Public is also a valued partner of our team.
